Referrals & Insurance
We want you to get all of the benefits you are eligible for from your health insurance coverage. Many health insurance plans require referral authorization from your primary care physician prior to consultation with a specialist. In addition, most plans also require pre-authorization before any type of surgical procedure or laser treatment. It is your responsibility to obtain the required referral authorization from your primary care physician. Failure to obtain a referral authorization, if required, will result in your health plan denying payment for our services. In this case, you will be personally financially responsible for all services obtained through our office.
If you are uncertain whether your health plan requires referral preauthorization, please call your plan’s customer service representative (number listed on the back of your health plan ID card) or your family physician’s office. We are participating physicians for Medicare, which allows us to bill Medicare directly. Because we also file all private insurance forms for the convenience of our patients, it is important that you bring your insurance cards with you at every visit so that we have accurate information about your coverage. Many plans require a co-payment. We are required to collect co-payments at the time of your visit. We will bill your plan directly for our services. After your plan has paid its part of the charges, we will send you a bill for your portion of the balance.
If you do not have medical insurance, you will be expected to pay for your consultation at the time of your visit. Payment for surgery must be arranged in advance. Our business office staff will work with you to set up a reasonable plan of payment so that you can receive appropriate treatment without delay. Our office accepts most major credit card payments through Visa, MasterCard, Discover.
